Tuberculosis clinical trials
As an institutional review board member, I saw a few clinical trials come through board review. There was a controversial trial that asked whether stable housing might improve HIV-related outcomes. There were trials related to diabetes prevention and anthrax vaccines. And there was a series of trials on preventing and treating tuberculosis. In 2007, I joined the Division of Tuberculosis Elimination, which sponsored those trials through the Tuberculosis Trials Consortium (TBTC). In that connection, I pursued methodological inquiries about surrogate markers for response to therapy and efficiently asking whether a new regimen is no worse than a known, effective regimen. And I developed a special appreciation for data standards to support registration-level research (like our trials) through traceable, transparent, and robust data workflows.

The analysis in this paper was led by Davie Chia while he was in an epidemiology elective during his medical training. It is my only published work that traces an analysis of ethics in a specific protocol aver the whole protocol’s arc, meant as a worked example for other IRB members.

I gave this paper its intentionally provocative title as a claim on both its rigor (see technical appendix) and its practical application to diagnostics in clinical trials. The work was carried collaboratively especially with Pei-Jean Feng — then new to tuberculosis — and Stefan Goldberg, TBTC project officer — both thought partners and friends. Some graphics found an additional home in 20130314_data-whispering.

TBTC Study 24 posed special challenges for administration and for analysis. This manuscript traces many of those challenges and carries a graphic that I developed to capture as much information as possible while remaining legible. Opinions differed on whether that information remained legible, but the graphic itself found a home in 20130314_data-whispering, 20151029_dashes-dots-small-data-graphics, and 20220419_composing-graphics-literary. The dashes-dot presentation including an earlier working version of the figure along with the published version. The composing-graphics presentation traced development through 13 versions to illustrate revision of a graphic as akin to revising a text. In the presenter’s notes, I wrote, “This graphic was revised so extensively in part to experiment with tradeoffs between information density and clarity. One coauthor objected to including this graphic in the manuscript. To be fair, since it was reduced in size, even an astute reader might have difficulty discerning fine details. At a decent size, however, I contend that it provides a useful amount of detail in a format that complements (or rivals!) a tabular format.”

I developed the simulation-based allocation forecasting method that this manuscript carried through to execution.
